New Patients!

Please make sure to bring Identification and your Insurance Information with you on your first visit. If you cannot provide your Insurance Information (i.e. your insurance member ID, found on most insurance's member card), AND your insurance is Denti-Cal of Medi-Cal or Delta Dental, our office can attempt to look your insurance information up through your SSN.

We will lookup your coverage as you fill out our New Patient paperwork out to make sure you're covered for the planned procedures and to avoid any unexpected charges to your account. If you are not completely covered for the treatment planned, we will make sure to inform you of that before rendering any treatment.

After making sure your insurance is in order and making sure your forms are properly filled out, we will call you back to our operating rooms to explain and begin treatment.

*Our new patient forms will take a bit of time to fill out on your first visit, as we need to be thorough with regards to our policies, practices, and your health history, so please arrive early if possible.
